In consideration of this entry being accepted, I, the undersigned, intending to be legally bound, hereby for myself, my heirs, executors and administrators, waive and release any and all rights and claims that I may have against Gifford Pinchot State Park and the PA DCNR, “Gulliver’s Run” Inc., The National Canine Cancer Foundation, all officials, officers, volunteers, sponsors or any other representatives for any injuries or damages that may be suffered by me or my dog in this event.
I hereby certify that my dog and I are physically fit and have sufficiently trained for the competition of this event. I further certify that my dog is licensed and that its vaccinations are current and that I can produce proof of these on the day of the event upon request. I acknowledge that any injuries sustained by me or my dog during the course of this event are caused by and arise out of risks of which I have full knowledge and which I fully assume. I understand that this event is a trail run and has a variety of natural obstacles which my dog and I must assume full responsibility for managing safely. I further certify that all information and or documentation provided in connection with this application is true and accurate as of the date of the scheduled event. I will be responsible for the conduct of my dog, for cleaning up after my dog, and for keeping my dog leashed and under control at all times.
I agree to indemnify and hold all sponsors of this event harmless against any and all claims and demands for any accident, injury, or damage whatsoever caused to any person or property by me or my dog arising, directly or indirectly, during the course of this event. I will additionally permit the free use by the sponsors of this event of our names, pictures or likeness in broadcasts, newsletters, or promotional materials.
The PAWS4POTTER event “Making A Mark On K9 Cancer” held on 10/21/17 at Thompson Park of Watertown, NY, is a dog-walking event to benefit pet cancer awareness via Paws4Potter and to benefit the National Canine Cancer Foundation (a 501c-3 non-profit organization).
As this is a dog-walking event, I (as a participant) understand that there may be associated risks to me or to my dog(s) by participating in this event, including but not limited to falling, interaction with other participants, dog-dog interaction, effects from the weather, and conditions of the park walkways. I hereby assume all risks, including personal injury to me or to my dog(s), property loss, and other damages that may be incurred by participating in this event.
It is my sole responsibility to dress appropriately for this event. I am solely responsible for my own health and safety, as well as the health and safety of my dog(s). I am physically fit and able to participate in this walking event. If walking with my dog(s), I represent that he/she/they are physically fit and able to participate as well and that he/she/they are current on Rabies vaccination and are not in heat at the time of the event.
I understand that PAWS4POTTER and Thompson Park are not responsible for any of the above described risks that may be incurred by my participation (or the participation of my dog(s)) at this event.
I grant permission to the organizers of this event to use my and/or my dog’s name and image as an event participant in photographs and videos.
I have read and understood this waiver and agree to its terms.
If the participant is under the age of 18 years, I – as the parent or legal guardian – hereby consent to his/her participation in this event and agree, on behalf of myself and the participant, to the terms of this waiver.
